does backing up the system state back up IIS metabase as well? What is the
best practice to back up IIS metabase along with regular backups?

System states includes metabase. but if you frequently perform changes you
would like to rollback, it is better to back it up separately to ease
restoration.
Additionally, you can also export registry-based configuration (using
reg.exe for example) and files (inetpub but also %windir%\system32\inesrv
and optional \LogFiles)
