How can I determine which application pool or site brought the server down?
IIS6 on Windows 2003 Server.

What do you mean by "brought the server down"? Windows itself? IIS?

In general, code running inside an application pool (w3wp.exe) can't cause
other processes on the machine to die, nor cause the entire server to BSOD.
Exceptions would involve you calling into kernel mode functionality etc.
